Job summary

Diablo Valley Post Acute in Concord, CA is seeking a licensed Physical Therapist (PT) to join our in-house rehabilitation team. We offer Full-Time, Part-Time, and PRN opportunities with flexible scheduling in a patient-centered, collaborative setting.

As a PT you will evaluate patients, design individualized plans, and work with a multidisciplinary team to restore independence and mobility. A CA PT license, strong clinical skills, and excellent communication are required.

Qualifications

  • Current California Physical Therapist (PT) license is required.
  • Strong clinical, communication, and interpersonal skills are essential.
  • Ability to work effectively in a collaborative interdisciplinary environment.

Responsibilities

  • Perform comprehensive physical therapy evaluations and develop individualized treatment plans.
  • Provide evidence-based interventions to improve mobility, strength, balance, coordination and independence.
  • Monitor patient progress and modify treatment plans based on clinical findings and patient response.
  • Supervise Physical Therapist Assistants in accordance with California State Practice Act requirements.
  • Collaborate with physicians, nursing staff, therapy professionals, and interdisciplinary care teams.
  • Participate in discharge planning and help patients transition safely to the next level of care.
  • Recommend appropriate durable medical equipment (DME) and assist patients and families with safe mobility strategies.
  • Educate patients and caregivers on home exercise programs, safety, and functional mobility.
  • Maintain accurate, timely and compliant clinical documentation.

What You'll Do

As a Physical Therapist, you'll help patients regain strength, mobility, balance, and confidence while developing individualized rehabilitation programs focused on maximizing independence and quality of life.

Key Responsibilities
