Job summary

Victorian Post Acute in San Francisco, CA is seeking a compassionate Physical Therapist (PT) to join our in-house therapy program. We offer flexible scheduling with Full-Time, Part-Time, and PRN opportunities and a competitive starting pay of $60-$65 per hour DOE.

New graduates are welcome, sign-on bonus available, and you will collaborate with a multidisciplinary team to evaluate patients, develop treatment plans, supervise PTAs, and support discharge planning while maintaining high-quality

Qualifications

  • Current Physical Therapist (PT) license in good standing.
  • Strong communication, organization, and interpersonal skills.
  • Passion for delivering exceptional patient-centered care.
  • Ability to work effectively within a multidisciplinary team.

Responsibilities

  • Evaluate patients and develop individualized treatment plans.
  • Provide skilled physical therapy services in accordance with physician orders and care plans.
  • Supervise Physical Therapist Assistants (PTAs) in compliance with state practice regulations.
  • Collaborate with interdisciplinary team members to coordinate and optimize patient care.
  • Participate in care conferences, rehabilitation meetings, and utilization reviews as needed.
  • Maintain timely and accurate documentation, including evaluations, treatment notes, progress reports, and discharge summaries.
  • Educate patients, families, and caregivers on treatment goals, mobility strategies, safety techniques, and home exercise programs.
  • Assist with discharge planning and recommendations for continued care.
  • Recommend and coordinate appropriate durable medical equipment (DME).
  • Ensure a safe treatment environment by monitoring and reporting equipment maintenance needs.

Key Responsibilities
