City View Post Acute in San Francisco, CA is seeking a Physical Therapist (PT) to join our in-house rehabilitation team. We offer Full-Time, Part-Time & PRN opportunities with pay $60–$63+ per hour depending on experience.

Qualifications

  • Current California Physical Therapist (PT) license or eligibility for licensure.
  • Strong clinical, communication, and problem-solving skills.
  • Excellent interpersonal and organizational abilities.

Responsibilities

  • Perform comprehensive physical therapy evaluations.
  • Develop and implement individualized treatment plans based on resident needs and goals.
  • Provide skilled therapeutic interventions to improve strength, balance, mobility, coordination, and overall function.
  • Monitor resident progress and modify treatment plans to maximize outcomes.
  • Collaborate with physicians, nursing staff, therapy professionals, and the interdisciplinary care team to coordinate resident care.
  • Educate residents and families on therapeutic exercises, mobility techniques, safety strategies, and fall prevention.
  • Accurately document evaluations, treatments, progress notes, and discharge planning.
  • Participate in care planning and interdisciplinary discussions as appropriate.
  • Build positive relationships with residents while providing compassionate, patient-centered care.
