Job summary

North Texas Therapy & Home Care in Plano, TX is seeking a licensed Physical Therapist to provide pediatric services in patients’ homes and daytime facilities. This therapist-owned practice focuses on individualized care plans and collaborative care with a multidisciplinary team.

Requirements include a graduate degree in physical therapy, TX license, pediatric experience, and auto insurance. Spanish bilingual a plus; home health experience preferred.

Qualifications

• Graduate degree in physical therapy from an accredited program.
• TX license to practice physical therapy.
• Pediatric experience is required (New grads: minimum 6 weeks pediatric OT internship).
• Home health experience preferred.

Responsibilities

• Provide skilled physical therapy services in patients' homes or daytime facilities for ages 0–21, 2+ visits per week.
• Determine degree and type of impairment using appropriate testing instruments.
• Interpret diagnostics and prepare written reports.
• Collaborate with the multidisciplinary team to plan and evaluate goals; instruct team in PT modalities.
• Modify care plans to meet therapeutic goals and progress.
• Educate staff, caregivers, and patients through training materials and in-service programs.

Requirements

• Graduate degree from an accredited academic program in physical therapy
• License to practice physical therapy in the state of Texas
• Experience in pediatrics required (New grads must have a minimum of 6 weeks of pediatric OT experience - can include internship)
• Home health experience preferred
• Bilingual in Spanish a plus
• Must carry valid auto insurance and drivers license.

Responsibilities

• Provide skilled physical therapy services in the home or daytime facility of patients 0-21 years of age, 2 or more times per week, in accordance with physician orders
• Define the patients degree and type of physical and gross motor impairment(s) by selecting and administering the most appropriate testing instruments
• Interpret diagnostic data and prepare written diagnostic reports
• In collaboration with the multi-disciplinary team, plan and evaluate team goals for each patient as well as advise, consult, and instruct the team in specific physical therapy modalities and interventions
• Plan for and modify care to meet goals of physical therapy interventions
• Educate staff, caregivers, and patients by designing and delivering training, printed material/resources and in-service programs in accordance with the total physical therapy program
• May provide supervision to Certified Physical Therapy Assistants
