Job summary

CommuniCare Rehab, a member of the CommuniCare Family of Companies, has a Full Time opening for a Physical Therapist at Grande Pointe Healthcare Center in Richmond Heights, OH. Our therapists are In-House Team Members, delivering direct impact with residents through therapy.

Responsibilities include evaluating patient charts, developing treatment plans, implementing therapy, supervising PTAs, and leading interdisciplinary meetings to share findings and advance care within a multi-center network.

Qualifications

• PT degree from an accredited program.
• Valid or pending State PT license.

Responsibilities

• Evaluate, review, and screen patient’s/resident’s medical charts and develop a treatment plan.
• Implement treatment plan through direct therapy and educate the plan of care to the rehab team, including supervision for PTAs.
• Attend and lead interdisciplinary meetings to relay clinical findings and make recommendations to the rehab team.
• Opportunity to work at multiple centers to serve a wide variety of patient needs.

Pay Rate: $35.00 - $60.00 USD Hourly

Qualifications:

• A Bachelor’s, Master’s, or Doctorate Degree in Physical Therapy from an Accredited Program.
• Hold a valid or pending State Physical Therapy License

Job Responsibilities include, but are not limited to:

• Evaluate, Review, and Screen patient’s/resident’s medical charts, evaluations under physician order and develop a treatment plan in accordance with patient’s needs.
• Implement treatment plan through direct treatment, as well as education of the plan of care to all team members, including providing necessary supervision for PTAs.
• Attend/Lead interdisciplinary meetings to relay clinical findings and make recommendations to other members of rehab team, as well as conducting staff/family training and education when appropriate.
