Job summary

CommuniCare Rehab, a member of the CommuniCare Family of Companies, has a PRN opening for a Physical Therapist at Hanover Healthcare Center, Massillon, OH. This position supports our residents by evaluating needs, developing treatment plans, and delivering direct therapy.

As an in-house team member, you will collaborate with nurses, physicians, and therapists across centers, with flexible scheduling and opportunities for CEU resources.

Qualifications

• Bachelor's, Master's, or Doctorate in Physical Therapy.
• Current state PT license or eligible.

Responsibilities

• Evaluate, review, and screen patient charts to develop a treatment plan.
• Implement treatment and educate the care plan to the rehab team.
• Supervise PTAs and provide staff/family training when appropriate.
• Lead interdisciplinary meetings to discuss patient care across centers.

Qualifications

A Bachelor's, Master's, or Doctorate Degree in Physical Therapy from an Accredited Program. Hold a valid or pending State Physical Therapy License

Job Responsibilities include, but are not limited to:

Evaluate, Review, and Screen patient's/resident's medical charts, evaluations under physician order and develop a treatment plan in accordance with patient's needs. Implement treatment plan through direct treatment, as well as education of the plan of care to all team members, including providing necessary supervision for PTAs. Attend/Lead interdisciplinary meetings to relay clinical findings and make recommendations to other members of rehab team, as well as conducting staff/family training and education when appropriate.
