Job summary

CommuniCare Rehab, a member of the CommuniCare Family of Companies, has a Full Time opening for a Physical Therapist (PT) at Cedars Healthcare Center, where our therapists are In-House Team Members. This is an exciting opportunity to impact residents through therapy.

Qualifications include a degree in Physical Therapy and a current or pending PT license. Responsibilities include evaluating patients, implementing treatment plans, supervising PTAs, and participating in interdisciplinary teams with

Qualifications

• Bachelor's, Master's or Doctorate in Physical Therapy from an accredited program.
• Current or pending state Physical Therapy license.
• Demonstrated competency in therapy evaluation, principles and modalities.

Responsibilities

• Evaluate and screen residents' charts and develop a treatment plan.
• Implement plan through direct treatment and education of the care team.
• Maintain accurate billing information daily per facility procedures.
• Lead interdisciplinary meetings and provide staff/family training when needed.

Qualifications:

A Bachelor's, Master’s, or Doctorate Degree in Physical Therapy from an Accredited Program.

Hold a valid or pending State Physical Therapy License

Demonstrate competency in therapy treatment, evaluation principles, techniques, and materials & equipment utilization. Attainable productivity expectations.

Job Responsibilities include, but are not limited to:

Evaluate, Review, and Screen patient’s/resident’s medical charts, evaluations under physician order and develop a treatment plan in accordance with patient’s needs.

Implement treatment plan through direct treatment, as well as education of the plan of care to all team members, including providing necessary supervision for PTAs.

Maintain accurate billing information in accordance with facility procedures daily.

Attend/Lead interdisciplinary meetings to relay clinical findings and make recommendations to other members of rehab team, as well as conducting staff/family training and education when appropriate.
