CommuniCare Rehab, a member of the CommuniCare Family of Companies, has a Full Time opening for a Physical Therapist (PT) at Grande Pointe Healthcare Center, where our therapists are in-house team members. This is an exciting opportunity to make a direct impact with residents through therapy.

Qualifications

• Bachelor's, Master’s, or Doctorate in Physical Therapy from an accredited program.
• Hold a valid or pending State PT license.

Responsibilities

• Evaluate, review, and screen patient charts and evaluations; develop a treatment plan.
• Implement treatment and educate the plan of care to the rehab team; supervise PTAs as needed.
• Attend/interdisciplinary meetings; relay clinical findings and make recommendations.

Qualifications

• A Bachelor's, Master’s, or Doctorate Degree in Physical Therapy from an Accredited Program.

Job Responsibilities

• Evaluate, Review, and Screen patient’s/resident’s medical charts, evaluations under physician order and develop a treatment plan in accordance with patient’s needs.
• Implement treatment plan through direct treatment, as well as education of the plan of care to all team members, including providing necessary supervision for PTAs.
• Attend/Lead interdisciplinary meetings to relay clinical findings and make recommendations to other members of rehab team, as well as conducting staff/family training and education when appropriate.
