Job summary

CommuniCare Rehab is hiring a Physical Therapist (PT) at Pleasant Valley Healthcare Center in West Virginia. This full-time role offers an in-house team environment with a focus on resident care and a generous sign-on bonus.

Responsibilities include evaluating patient charts, developing and implementing a treatment plan, and leading education for the rehab team. The position offers work-life balance, PTO, and opportunities to work across multiple centers.

Qualifications

• Bachelor's/Master's/Doctorate in Physical Therapy from an accredited program.
• Valid or pending State Physical Therapy License.

Responsibilities

• Evaluate and screen residents' medical charts under physician order and develop a treatment plan.
• Implement treatment plan directly and educate the plan of care to the rehab team, supervising PTAs as needed.
• Attend or lead interdisciplinary meetings, relay findings, and provide staff/family training when appropriate.

Qualifications:

• A Bachelor's, Master’s, or Doctorate Degree in Physical Therapy from an Accredited Program.
• Hold a valid or pending State Physical Therapy License

Job Responsibilities include, but are not limited to:

• Evaluate, Review, and Screen patient’s/resident’s medical charts, evaluations under physician order and develop a treatment plan in accordance with patient’s needs.
• Implement treatment plan through direct treatment, as well as education of the plan of care to all team members, including providing necessary supervision for PTAs.
