Job summary

Chandler Creek Post Acute in Greer, SC, is seeking a dedicated Physical Therapist (PT) to join our full-time rehab team on the day shift. You will evaluate residents, design individualized treatment plans, and help patients regain mobility and independence in a caring, small-community setting.

Qualifications

• Familiarity with PointClickCare and Casamba preferred.
• Strong communication and clinical decision-making skills.

Responsibilities

• Conduct patient evaluations and develop customized treatment plans.
• Provide therapeutic interventions to improve strength, mobility, and function.
• Monitor patient progress and adjust treatment plans as needed.
• Educate residents and families on therapy techniques and goals.
• Maintain accurate and timely clinical documentation.
• Collaborate with interdisciplinary teams to ensure optimal patient outcomes.
