Job summary

Piedmont Post Acute is an 88-bed skilled nursing facility in Piedmont, South Carolina—20 minutes from Greenville. We are seeking a compassionate and skilled Physical Therapist (PT) to join our rehab team to help residents reach their highest level of independence.

We offer competitive pay: $45-$55/hour, a 401(k) with company match, PTO & sick time (for eligible employees), and a flexible PRN schedule with consistent hours.

Qualifications

• Active, unencumbered Physical Therapist license in South Carolina.
• Degree in Physical Therapy (Bachelor's, Master’s, or Doctorate).
• Experience in long-term care or rehab setting preferred.
• Familiarity with PCC and Casamba preferred.
• Strong communication and clinical skills.
• Passion for improving resident outcomes.

Responsibilities

• Evaluate and treat residents to improve mobility, strength, and function.
• Develop individualized treatment plans.
• Provide therapeutic interventions to improve mobility, strength, and balance.
• Monitor progress and adjust plans as needed.
• Maintain accurate and timely documentation.
• Collaborate with interdisciplinary team members.
• Educate residents and families on treatment plans and goals.

Position Summary
