Job summary

Continuum Therapy Partners is seeking a Physical Therapist to join our team in a PRN daytime capacity at Avalon Health & Rehab Center, Mt. Vernon Ohio.

You will evaluate and treat patients, develop plans of care in collaboration with other disciplines, and educate patients and families on home exercise programs and injury prevention. A DPT, current state license, and NPTE are required; one year of experience is preferred.

Qualifications

• DPT required.
• Current, unencumbered state Physical Therapy License; NPTE required.
• 1 year of experience as a Physical Therapist preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.

Physical Therapist Requirements & Responsibilities

• Doctor of Physical Therapy (DPT) degree required
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
