Job summary

Continuum Therapy Partners is seeking a Physical Therapist to provide therapist-led care across our growing national footprint. You will evaluate, design, and implement individualized treatment plans in coordination with other disciplines, and monitor patient progress to adjust plans of care as needed.

The role requires a Doctor of Physical Therapy, an active state license, and NPTE eligibility, with a preference for 1 year of prior PT experience.

Qualifications

• DPT degree is required.
• Active, unencumbered state PT license; NPTE must be passed.
• 1 year of PT experience preferred.

Responsibilities

• Evaluate, develop, and implement individualized treatment plans in coordination with other disciplines.
• Document patient progress and adjust plans of care based on outcomes.
• Educate patients and families on home exercise programs and injury prevention.

Physical Therapist Requirements & Responsibilities

• Doctor of Physical Therapy (DPT) degree required
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
