Job summary

Addus HomeCare in Camden, TN is seeking a PRN Physical Therapist to join our home health team. You will evaluate patients, develop plans, and deliver one-on-one care in patients' homes, helping improve mobility and independence.

This PRN role offers flexible scheduling (Mon-Fri, as needed), with the support of a multidisciplinary team, competitive pay, and opportunities for growth. A Tennessee PT license and CPR certification are required, with home health experience preferred.

Qualifications

• Graduate of an accredited Physical Therapy program approved by the APTA.
• Current Tennessee PT license.
• Home health experience preferred but not required.
• CPR certification current.
• Valid driver’s license and reliable transportation with proof of automobile insurance.

Responsibilities

• Evaluate patients and develop individualized treatment plans based on physician orders.
• Provide skilled physical therapy services in the patient’s home.
• Help patients improve mobility, strength, balance, and functional independence.
• Educate patients, families, and caregivers on treatment plans and home exercise programs.
• Recommend adaptive equipment and home modifications to promote safety and independence.
• Collaborate with physicians and the interdisciplinary care team to achieve optimal patient outcomes.
• Complete accurate and timely clinical documentation and care plan updates.
• Supervise Physical Therapist Assistants (PTAs) in accordance with company policy and state regulations.
• Encourage and motivate patients throughout their rehabilitation journey.
