Job summary

Addus HomeCare in Camden, TN, is seeking a PRN Physical Therapist to deliver one-on-one home health care. You’ll assess patients, develop personalized plans, and provide skilled therapy in patients’ homes with a flexible PRN schedule.

This role emphasizes improving mobility and independence while coordinating care with physicians and the interdisciplinary team. The ideal candidate holds an APTA-accredited PT program background, Tennessee license, CPR, and reliable transportation.

Qualifications

• Graduate of an accredited Physical Therapy program approved by the American Physical Therapy Association (APTA).
• Current Physical Therapist (PT) license in the state of Tennessee.
• Home Health experience preferred, but not required.
• Current CPR certification.
• Valid driver’s license, reliable transportation, and proof of automobile insurance.

Responsibilities

• Evaluate patients and develop individualized treatment plans based on physician orders.
• Provide skilled physical therapy services in the patient’s home.
• Help patients improve mobility, strength, balance, and functional independence.
• Educate patients, families, and caregivers on treatment plans, home exercise programs, and safety techniques.
• Recommend adaptive equipment and home modifications to promote safety and independence.
• Collaborate with physicians and the interdisciplinary care team to achieve optimal patient outcomes.
• Complete accurate and timely clinical documentation and care plan updates.
• Supervise Physical Therapist Assistants (PTAs) in accordance with company policy and state regulations.
• Encourage and motivate patients throughout their rehabilitation journey.
