Job summary

Addus HomeCare in Camden, TN is hiring a Full-Time Physical Therapist to provide one-on-one home health services. You will assess patients, develop treatment plans, and help them regain mobility and independence while complying with physician orders and state regulations.

This role includes regular weekday visits with travel, on-call rotation, competitive pay, 401(k) match, generous PTO, and a supportive team culture focused on delivering high-quality patient care.

Qualifications

• Graduate of an accredited Physical Therapy program (APTA approved).
• Current Physical Therapist license in TN.
• Home Health experience preferred, but not required.
• Two years in acute care or similar setting preferred.
• Current CPR and First Aid certification.

Responsibilities

• Evaluate patients and develop individualized treatment plans based on physician orders.
• Provide skilled PT services per plan of care and state regulations.
• Improve mobility, strength, balance, and independence.
• Educate patients and families on treatment plans and home exercise programs.
• Recommend adaptive equipment and home modifications for safety.
• Collaborate with physicians and care team to optimize outcomes.
• Complete accurate documentation and care plan updates.
• Supervise PTAs according to policy and state regulations.
• Encourage and motivate patients throughout rehabilitation.

Schedule: Monday�Friday | 8:00 AM � 4:30 PM- Regular weekday patient visits with participation in a rotating on-call schedule, including evenings and weekends. Daily hours vary based on patient needs, travel, and documentation.

Key Responsibilities

• Evaluate patients and develop individualized treatment plans based on physician orders
• Provide skilled physical therapy services in accordance with the physician’s plan of care and state regulations
• Help patients improve mobility, strength, balance, and functional independence
• Educate patients, families, and caregivers on treatment plans, home exercise programs, and safety techniques
• Recommend adaptive equipment and home modifications to promote safety and independence
• Collaborate with physicians and the interdisciplinary care team to achieve optimal patient outcomes
• Complete accurate and timely clinical documentation and care plan updates
• Supervise Physical Therapist Assistants (PTAs) in accordance with company policy and state regulations
• Encourage and motivate patients throughout their rehabilitation journey

Qualifications

• Graduate of an accredited Physical Therapy program approved by the American Physical Therapy Association (APTA)
• Current Physical Therapist (PT) license in the state of Tennessee
• Home Health experience preferred, but not required
• Two (2) years of experience in an acute care or similar clinical setting preferred
• Current CPR and First Aid certification
• Valid driver’s license, reliable transportation, and proof of automobile insurance
