Job summary

Addus HomeCare in Tennessee is seeking a dedicated Physical Therapist to join our Home Health team on a full-time basis. You will deliver PT services per physician plans of care, evaluating function and developing personalized care plans for patients in their homes.

Qualifications

• Graduate of an APTA-accredited Physical Therapy program.
• Current PT license in the state of practice.
• Two years of acute care or similar setting experience preferred.
• Current CPR and First Aid certification.
• Reliable transportation and valid driver’s license.

Responsibilities

• Provide PT services according to physician plan of care and state guidelines.
• Evaluate patient function and develop individualized care plans.
• Educate patients, families, and caregivers on treatment plans and activities.
• Translate prescribed exercises into daily living activities.
• Recommend equipment/home modifications to support safety and independence.
• Collaborate with physicians and the interdisciplinary team to coordinate care.
• Document notes, progress reports, and summaries in a timely manner.
• Support and motivate patients and families throughout rehabilitation.
• Supervise PT Assistants per policy and state regulations.
• Maintain accurate clinical records and track patient goals.

What You’ll Do

• Provide physical therapy services according to a physician’s plan of care and state practice guidelines
• Evaluate patient function and assist in developing individualized care plans
• Educate patients, families, and caregivers on treatment plans and therapeutic activities
• Translate prescribed exercises into functional activities and activities of daily living
• Recommend modifications to equipment and home environments to support patient safety and independence
• Collaborate with physicians and interdisciplinary team members to coordinate care and monitor patient progress
• Document clinical notes, progress reports, and summaries in a timely and accurate manner
• Support and motivate patients and families throughout the rehabilitation process
• Supervise Physical Therapist Assistants in accordance with company policy and state regulations
• Maintain accurate clinical records and track patient goals and outcomes

Qualifications

• Graduate of an accredited Physical Therapy program approved by the American Physical Therapy Association (APTA)
• Current Physical Therapist (PT) license in the state of practice
• At least two (2) years of experience in an acute care or similar setting (preferred)
• Current CPR and First Aid certification
• Reliable transportation, valid driver’s license, and current automobile insurance
