Job summary

Addus HomeCare in Camden, TN is seeking a PRN Physical Therapist to deliver in-home rehabilitation. You will assess patients, develop plans, and provide one-on-one PT services in patients' homes, aiming to improve mobility and independence.

This role offers flexible scheduling and the opportunity to work closely with physicians and the care team. Ideal candidates will hold a Tennessee PT license, CPR certification, a valid driver’s license, and prefer Home Health experience.

Qualifications

• Graduate of an accredited Physical Therapy program approved by the APTA.
• Current Tennessee PT license.
• CPR certification is current.

Responsibilities

• Evaluate patients and develop individualized treatment plans based on physician orders.
• Provide skilled physical therapy services in the patient’s home.
• Help patients improve mobility, strength, balance, and functional independence.
• Educate patients, families, and caregivers on treatment plans, home exercise programs, and safety techniques.
• Recommend adaptive equipment and home modifications to promote safety and independence.
• Collaborate with physicians and the interdisciplinary care team to achieve optimal patient outcomes.
• Complete accurate and timely clinical documentation and care plan updates.
• Supervise PTAs in accordance with company policy and state regulations.
• Encourage and motivate patients throughout their rehabilitation journey.

Education

Graduate of an accredited Physical Therapy program

Current Physical Therapist license in Tennessee

Current CPR certification

• Current Physical Therapist (PT) license in the state of Tennessee
• Home Health experience preferred, but not required
• Current CPR certification
• Valid driver�s license, reliable transportation, and proof of automobile insurance
