Job summary

Tennessee Quality Care in Camden, TN is seeking a full-time Physical Therapist to deliver one-on-one home health care. You will evaluate patients, create individualized plans, and provide skilled PT services in patients’ homes to improve mobility and independence.

The role emphasizes collaboration with physicians and an interdisciplinary team, documentation accuracy, and patient education. A valid TN PT license and CPR/First Aid are required, with on-site visits and rotating on-call duties.

Qualifications

• Graduate of an accredited Physical Therapy program approved by the APTA.
• Current PT license in Tennessee required.
• Home health experience preferred but not required.
• CPR and First Aid certification required.

Responsibilities

• Evaluate patients and develop individualized treatment plans based on physician orders.
• Provide skilled physical therapy services in accordance with the physician’s plan of care and state regulations.
• Help patients improve mobility, strength, balance, and functional independence.
• Educate patients, families, and caregivers on treatment plans, home exercise programs, and safety techniques.
• Recommend adaptive equipment and home modifications to promote safety and independence.
• Collaborate with physicians and the interdisciplinary care team to achieve optimal patient outcomes.
• Complete accurate and timely clinical documentation and care plan updates.
• Supervise Physical Therapist Assistants (PTAs) in accordance with company policy and state regulations.
• Encourage and motivate patients throughout their rehabilitation journey.

Schedule: Monday�Friday | 8:00 AM � 4:30 PM- Regular weekday patient visits with participation in a rotating on-call schedule, including evenings and weekends. Daily hours vary based on patient needs, travel, and documentation.

Qualifications

• Graduate of an accredited Physical Therapy program approved by the American Physical Therapy Association (APTA)
• Current Physical Therapist (PT) license in the state of Tennessee
• Home Health experience preferred, but not required
• Two (2) years of experience in an acute care or similar clinical setting preferred
• Current CPR and First Aid certification
• Valid driver’s license, reliable transportation, and proof of automobile insurance
