Job summary

Tennessee Quality Care is seeking a dedicated Full-Time Physical Therapist to deliver one-on-one home health services, helping patients regain mobility and independence. You will collaborate with an interdisciplinary team to implement individualized plans aimed at optimal outcomes.

Ideal candidates have a PT degree, Tennessee license, CPR/First Aid, and 2+ years in acute care. Home health experience preferred. A valid driver’s license and reliable transportation are required.

Qualifications

• Graduate of an accredited Physical Therapy program approved by the American Physical Therapy Association (APTA)
• Current Physical Therapist (PT) license in the state of Tennessee
• Home Health experience preferred, but not required
• Two (2) years of experience in an acute care or similar clinical setting preferred
• Current CPR and First Aid certification
• Valid driver’s license, reliable transportation, and proof of automobile insurance

Responsibilities

• Evaluate patients and develop individualized treatment plans based on physician orders
• Provide skilled physical therapy services in accordance with the physician’s plan of care and state regulations
• Help patients improve mobility, strength, balance, and functional independence
• Educate patients, families, and caregivers on treatment plans, home exercise programs, and safety techniques
• Recommend adaptive equipment and home modifications to promote safety and independence
• Collaborate with physicians and the interdisciplinary care team to achieve optimal patient outcomes
• Complete accurate and timely clinical documentation and care plan updates
• Supervise Physical Therapist Assistants (PTAs) in accordance with company policy and state regulations
• Encourage and motivate patients throughout their rehabilitation journey

Schedule: Monday�Friday | 8:00 AM � 4:30 PM- Regular weekday patient visits with participation in a rotating on-call schedule, including evenings and weekends. Daily hours vary based on patient needs, travel, and documentation.
