Job summary

AccessHealth is seeking a Physical Therapist to provide personalized, in-home and senior living care to older adults, using evidence-based treatments to improve mobility, independence and quality of life.

You will conduct evaluations, develop care plans, deliver one-on-one therapy, document in EMR, and collaborate with interdisciplinary teams, including telehealth to extend access. This role offers full-time in-home work with opportunities for continuing education and career growth.

Qualifications

• Must hold a valid Physical Therapy license or be eligible in the practice state.
• Degree from an accredited Physical Therapy program.
• Proficiency in computer use and EMR systems.
• Current CPR certification.
