Job summary

Plan A Health in New Jersey is seeking a licensed Physical Therapist to provide in-home, senior living, and virtual care to older adults, focusing on one-on-one sessions. You will evaluate patients, develop personalized treatment plans, document progress in EMR, and collaborate with a multidisciplinary team to support mobility, independence, and aging in place.

Qualifications

• Valid Physical Therapy license or eligibility in practice state(s).
• Degree from an accredited PT program; proficiency with EMR systems.
• Current CPR certification.

Job description

Role:

Licensed Physical Therapist providing in-home, senior living, and virtual care to older adults, primarily through one-on-one sessions.

Responsibilities:

• Conduct evaluations, develop personalized treatment plans, and deliver evidence-based therapy to improve mobility, independence, and quality of life.


• Document patient progress accurately within EMR systems and collaborate with interdisciplinary teams including PTs, OTs, SLPs, and Fitness Specialists.


• Educate patients, caregivers, and staff to promote safety and aging in place.


• Incorporate telehealth and virtual care for increased flexibility and patient access.

Qualifications & Skills:

Physical Requirements:

Ability to exert moderate effort, assist with patient transfers, perform CPR, and drive to various care settings.
