Job summary

Azza HealthCare Agency is seeking a licensed Physical Therapist to deliver in-home and senior living care in Sioux City, IA, serving older adults with mobility and independence goals.

You will evaluate, plan, and administer evidence-based therapies, coordinate with interdisciplinary teams, and document care in EMR. Telehealth may be incorporated as appropriate; H1B sponsorship not specified.

Qualifications

• Licensed Physical Therapist or eligible to practice in the US.
• Degree from an accredited PTA/PT program.
• Proficiency with computers and EMR systems.
• Current CPR certification.

Responsibilities

• Provide one-on-one therapy in patients' homes and senior communities.
• Perform evaluations, create care plans, and deliver evidence-based treatments.
• Support patient mobility, safety, and independence.
• Accurately document care in EMR systems.

Position Overview:

A licensed Physical Therapist responsible for delivering personalized in-home and senior living care to older adults, utilizing evidence-based treatments to improve mobility, independence, and quality of life.

Key Responsibilities

• Provide one-on-one therapy in private residences and senior communities
• Conduct evaluations, develop care plans, and deliver evidence-based treatments
• Support patient wellness, mobility, and functional independence
• Document care accurately in EMR systems
• Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and staff on safety and aging in place
• Incorporate telehealth and virtual care as appropriate

Qualifications & Skills

• Valid Physical Therapy license or eligibility
• Degree from an accredited program
• Proficiency in computer use and EMR systems
• Current CPR certification

Physical Requirements:

Ability to perform moderate to heavy physical activities, including lifting, repositioning patients, walking, climbing stairs, and driving, with adherence to PPE protocols.
