Job summary

CONSUMER DIRECTED SERVICES INC. seeks a Licensed Physical Therapist to provide in-home, senior living, and virtual care to older adults in South Carolina.

You will evaluate patients, develop personalized plans, and deliver evidence-based therapy to improve mobility, independence, and wellness. You'll document progress in EMR, collaborate with interdisciplinary teams, educate patients and caregivers, and adapt with telehealth when appropriate.

Qualifications

• Valid PT license in the United States.
• Degree from an accredited physical therapy program.
• Proficiency with EMR systems and computers.
• Current CPR certification.
• Ability to transfer and reposition patients.
• Valid driver license and ability to travel to patient locations.
• Comfort wearing PPE.

Job description

Role:

Licensed Physical Therapist providing in-home, senior living, and virtual care to older adults.

Responsibilities:

Conduct evaluations, develop personalized treatment plans, deliver evidence-based therapy to enhance mobility, independence, and wellness. Document patient progress in EMR, collaborate with interdisciplinary teams, educate patients and caregivers, and incorporate telehealth when appropriate.

Qualifications & Skills:

Valid PT license, degree from an accredited program, proficiency with EMR and computers, current CPR certification. Physical ability to perform transfers, reposition patients, and drive to patient locations. Capable of exerting physical effort and wearing PPE.
