Job summary

Health Care Administrators Association® (HCAA) is seeking a Physical Therapist to deliver in-home and senior living care to older adults, with flexibility for full-time, part-time, or PRN work. You will perform evaluations, develop individualized care plans, and provide evidence-based therapies at patient residences and senior communities.

You will document care in electronic health records, collaborate with PTs, OTs, SLPs, and Fitness Specialists, and educate patients and caregivers to promote

Qualifications

• PT license or eligibility in practice states
• Accredited PT degree from an approved program
• Proficiency with electronic health records (EHR) and computers
• Current CPR certification

Responsibilities Include

• Providing one-on-one physical therapy in private residences and senior communities
• Performing evaluations, developing individualized care plans, and delivering evidence-based treatments

Physical Requirements

• Ability to exert medium physical effort and assist with patient transfers
• Perform CPR and provide assistance with mobility and repositioning
• Drive to patient locations and wear PPE as needed
