Job summary

Advocate Health Care seeks an experienced Physical Therapist to interpret referrals, evaluate patients, and implement individualized plans of care. The role requires a Doctorate in Physical Therapy and state licensure with BLS certification, plus 5–7 years of experience in rehab.

You will collaborate with physicians and staff, monitor outcomes, ensure documentation accuracy, and contribute to program development and clinical guidelines.

Qualifications

• Doctorate Degree in Physical Therapy from an accredited school.
• Licensed Physical Therapist in the state you work in; BLS for Healthcare Providers.
• 5–7 years of experience in physical therapy; hospital or outpatient setting.
• Advanced certification in a rehabilitation specialty as determined by Rehab leadership.

Responsibilities

• Interprets referrals and conducts musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations using appropriate methods.
• Develops and implements an individualized plan of care with treatment based on evaluation, goals, and techniques.
• Monitors outcomes, updates physicians and care team, and adjusts treatment plans as needed.
• Maintains productivity standards and adheres to billing and documentation policies and timelines.
• Mentors staff, develops clinical guidelines, and supports student clinical education.
