Job summary

Continuum Therapy Partners, LLC is seeking a dedicated Physical Therapist to join our PRN team. This part-time daytime role supports Avalon Health and Rehab in Mt. Vernon, OH, offering flexible scheduling while delivering high-quality patient care.

Requirements include a Doctor of Physical Therapy, an active state license and NPTE, plus preferred 1 year of experience. You will evaluate patients, develop plans of care, document progress, and educate families on home exercises.

Qualifications

• DPT degree required.
• Current, unencumbered state license; NPTE must be passed.
• 1 year of PT experience preferred.
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.

Physical Therapist Requirements & Responsibilities

• Doctor of Physical Therapy (DPT) degree required
