Job summary

Continuum Therapy Partners, LLC is seeking a Physical Therapist to join our Avon, OH team at The Woods on French Creek. A DPT is required and you will provide patient-centered evaluations, develop treatment plans, and coordinate care with other disciplines.

You will educate patients and families on home exercise programs, monitor progress, and adapt plans based on outcomes. Full-time daytime schedule with comprehensive benefits is offered.

Qualifications

• Doctor of Physical Therapy (DPT) degree required.
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE).
• 1 year of previous experience as a Physical Therapist preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
