Continuum Therapy Partners, LLC is seeking a Physical Therapist to join our team at Wayne Health & Rehab Center in Huber Heights, OH. The role focuses on evaluating patients, creating and adjusting treatment plans, and coordinating care with other disciplines.

The successful candidate will hold a DPT, possess an active state license and NPTE eligibility, and prefer 1 year of prior experience in physical therapy. A supportive leadership team prioritizes professional growth and patient outcomes.

Qualifications

• Doctor of Physical Therapy (DPT) degree required
• Current, unencumbered state Physical Therapy License; must pass the NPTE
• 1 year of previous experience as a Physical Therapist preferred
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
• Documents patient progress and adjusts plans of care based on clinical outcomes
• Educates patients and families on home exercise programs and injury prevention
