Job summary

Continuum Therapy Partners, LLC is seeking a licensed Physical Therapist to join our expanding team at Avalon Health and Rehab. The role focuses on evaluating patients, developing individualized treatment plans, and coordinating care with multidisciplinary teams.

The ideal candidate holds a DPT, has an active PT license, and preferably 1 year of experience in therapy. A strong commitment to patient outcomes and education is essential; sign-on bonus available for part-time positions.

Qualifications

• Doctor of Physical Therapy (DPT) degree required.
• Current, unencumbered state Physical Therapy License; must pass the NPTE.
• 1 year of previous experience as a Physical Therapist preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
• Documents patient progress and adjusts plans of care based on clinical outcomes
• Educates patients and families on home exercise programs and injury prevention
