Job summary

LiveWell Homecare Agency in Atlanta, GA seeks a Physical Therapist to evaluate and treat patients in Rehab and LTAC hospitals. You will develop and implement treatment plans, document progress, educate families, and coordinate with physicians.

Requirements include a Bachelor's in Physical Therapy (Doctorate preferred), at least 1 year in Rehab/SNF/LTAC/Acute settings, and active GA PT license or eligibility. BLS certification is required.

Qualifications

• Bachelor's degree in Physical Therapy is required.
• Doctoral Degree is preferred.
• Minimum 1 year experience in Rehab, SNF, LTACH, or Acute settings preferred.
• Current BLS certification and GA PT licensure or eligibility.

This position involves the evaluation and treatment of patients primarily in Rehab and LTAC hospitals, with responsibilities including conducting initial and discharge assessments, developing and implementing treatment plans, and documenting progress in the EMR. The role also includes educating patients, families, and staff, communicating patient progress within clinical teams, and coordinating with outside physicians and vendors for equipment needs.
