Job summary

Azza HealthCare Agency in Savannah, GA seeks a licensed Physical Therapist to evaluate and treat patients in rehab and LTAC settings, develop goals and treatment plans, and document progress daily. You will educate patients and families, mentor interns, and collaborate with an interdisciplinary team while coordinating equipment orders and vendor communications.

A Bachelor's in PT is required, Doctoral degree preferred, GA license or eligibility, and BLS certification; 1 year rehab experience

Qualifications

• Bachelor's Degree in Physical Therapy required.
• GA Physical Therapy license or eligibility, with BLS certification from the American Heart Association.
• Minimum of 1 year experience in rehab, SNF, LTACH, or acute care settings preferred.
• Basic computer literacy required.

Responsibilities

• Conduct initial and discharge evaluations, set goals, and develop treatment plans.
• Delivers timely, goal-oriented therapy to meet functional objectives.
• Identifies educational needs and provides PRN therapy.
