Job summary

Live Well Homecare is seeking a Physical Therapist to evaluate and treat patients primarily in rehab and LTACH hospital settings. You will conduct initial and discharge assessments, develop and implement treatment plans, and document patient progress in the EMR to support functional goals.

The role requires effective communication with an interdisciplinary team, outside physicians, and vendors, plus mentoring interns and participating in hospital-specific skills checks.

Qualifications

• Bachelor's Degree in Physical Therapy; Doctoral Degree preferred.
• At least 1 year of Rehab, SNF, LTACH, or Acute care experience.
• GA PT license or eligibility, BLS certification.
• Basic computer literacy.

Responsibilities

• Perform assessments, set goals, and document treatment plans and progress.
• Develop and deliver timely, goal-oriented therapy interventions.
• Identify educational needs and provide PRN therapy.
• Participate as a collaborative team member and serve as a mentor or CI for interns.
• Engage in hospital-specific skills checks and training.
