Job summary

LiveWell Homecare Agency, LLC seeks a licensed Physical Therapist to evaluate and treat patients in rehab and LTAC settings. You will develop and implement treatment plans, document daily progress, and educate patients and staff as part of an interdisciplinary team.

Responsibilities include conducting initial and discharge evaluations, mentoring interns when needed, and participating in skills assessments for the hospital and discipline. GA license and current BLS are required or eligibility.

Qualifications

• Bachelor’s Degree in Physical Therapy required; Doctoral Degree preferred.
• Minimum one year of experience in Rehab, SNF, LTACH, or Acute settings preferred.
• Current BLS certification, GA PT license or eligibility.

• Develop and deliver effective treatment to meet patient functional goals.
• Assess educational needs and provide PRN therapy as needed.
