Job summary

LiveWell Homecare Agency in Georgia seeks a Physical Therapist to evaluate and treat patients in Rehab and LTAC settings, develop and implement treatment plans, and document progress in the EMR. You will educate patients, families, and staff, and communicate patient status during clinical conferences.

You will coordinate equipment and vendor contacts, collaborate with outside physicians and interdisciplinary team members, and mentor interns while contributing to hospital and discipline-specific

Qualifications

• Bachelor’s Degree in Physical Therapy required; Doctoral Degree preferred.
• GA Physical Therapy license or eligibility and BLS certification.
• Minimum of 1 year experience in rehab settings such as SNF, LTACH, or acute care preferred.

Responsibilities

• Perform initial and discharge evaluations, set treatment goals, and document appropriately.
• Develop and deliver effective therapy treatments to meet functional objectives.
• Identify educational needs and provide PRN therapy.
• Participate as an interdisciplinary team member and mentor for interns.
• Engage in hospital and discipline-specific skills assessments.
