Job summary

Live Well Homecare is seeking a Physical Therapist to provide evaluation and treatment for patients in Rehab and LTAC hospitals, aiming to improve outcomes and functional independence.

The role includes developing plans, setting goals, documenting progress, and educating patients and families. It requires GA licensure eligibility and BLS, with preference for 1 year rehab experience in SNF/LTACH/acute care. This is a hands-on, mentorship-oriented clinical position.

Qualifications

• Licensed or eligible for licensure as a Physical Therapist in Georgia.
• Current BLS certification.
• Minimum 1 year experience in rehab, SNF, LTACH, or acute care settings preferred.

Responsibilities

• Conducts initial and discharge evaluations, develops treatment plans, and documents progress in the EMR.
• Sets treatment goals and delivers therapy to meet functional objectives in a timely manner.
• Educates patients, families, and staff as needed, and communicates patient progress during clinical conferences.
• Collaborates with interdisciplinary team members and participates in skills checks and training.
• Serves as a mentor for interns and manages equipment orders and vendor communications.
