Job summary

The Huntington Valley Healthcare Center is seeking a Staff Physical Therapist to evaluate and treat residents, and to document services in medical records. You will develop individualized plans to improve mobility and independence and educate residents and families about goals and home programs.

Supervising PT assistants, coordinating with physicians and staff, and ensuring compliance with safety, infection control, and facility policies are essential parts of the role.

Qualifications

• Bachelor's degree in Physical Therapy and licensure in the state.
• Ability to read and implement clinical procedures and guidelines.
• Proficiency with electronic health records and therapy documentation.

Responsibilities

• Evaluate residents to determine physical therapy needs and rehab potential.
• Develop and update individualized treatment plans to improve mobility and independence.
• Supervise PT assistants and coordinate with physicians and staff.
• Provide in-services and training for facility staff on rehabilitation programs.
• Maintain timely, accurate documentation and ensure safety and infection control.
