Job summary

Huntington-Valley-Healthcare-Center is seeking a Staff Physical Therapist to evaluate residents, develop tailored treatment plans, and improve mobility and function. You will lead PT assistants, coordinate with physicians and families, and ensure compliant documentation throughout care episodes.

Required is a state PT license, a degree in physical therapy, CPR (as applicable), and proficiency with Net Health and PointClickCare.

Qualifications

• Bachelor’s degree in Physical Therapy; Masters/Doctorate preferred.
• Licensed PT in the state.
• CPR certification, if required by licensure.
• Proficient with Net Health and PointClickCare for documentation.

Responsibilities

• Evaluate residents to determine PT needs and rehabilitation potential.
• Develop, implement, and update individualized treatment plans to improve mobility and function.
• Supervise Physical Therapy Assistants and other staff.
• Maintain timely, accurate documentation per policies and payer requirements.
• Educate residents and families on home programs, safety techniques, and fall prevention.
• Participate in care conferences and rehabilitation meetings.
