Responsibilities

• Evaluate residents to determine PT needs and rehab potential.
• Develop and update individualized treatment plans to improve mobility and independence.
• Supervise PT assistants and trainees in resident care.
• Communicate progress with physicians, nursing staff, families and interdisciplinary team.
• Document services timely and accurately in medical records.
