Job summary

Reliant Rehabilitation is seeking a licensed physical therapist to provide therapy services in skilled nursing facilities. You will screen and evaluate patients, develop and adjust treatment plans, and coordinate discharge planning while maintaining high standards of patient care and regulatory compliance.

You will work with a team of therapists to ensure quality care and effective utilization of resources, with ongoing opportunities for professional growth and leadership within the organization.

Qualifications

Graduate of an APTA-accredited school of physical therapy.
Valid physical therapist state license.
Strong interaction with facilities and patients in clinical settings.

Responsibilities

Provide physical therapy and related services for patients in facilities.
Develop initial and ongoing treatment plans of care and discharge plans.
Supervise assistants and Rehab Techs according to policy and state regulations.
Maintain professional conduct and ensure compliance with regulations.
