Reliant Rehabilitation is seeking a dedicated Physical Therapist to deliver therapy services in skilled nursing facilities. You will assess and treat patients, develop care plans, and discharge strategies while collaborating with facilities to ensure quality outcomes.

The role requires a current PT license and a commitment to professional growth, with ongoing education supported by the company.

Qualifications

• Graduate of an APTA-accredited school of physical therapy.
• Holds a valid physical therapist state license.

Responsibilities

• Provide physical therapy and related services to patients while upholding Reliant Rehabilitation guidelines.
• Develop initial and ongoing treatment plans, discharge plans, and case management.
• Supervise assistants and Rehab Techs; ensure quality patient care and compliance with regulations.
• Optimize patient function through direct therapy and supervision of PTAs where appropriate.
• Maintain professional conduct and ongoing clinical development through education.
