Job summary

Rockport Healthcare Services seeks a licensed Physical Therapist to provide direct patient care and lead a rehab team in a facility setting. You will evaluate patients, design and implement treatment plans, and document outcomes in the medical records.

Collaboration with physicians and families is essential for optimal recovery. Candidates should have a DPT or higher and current California licensure; prior geriatric rehabilitation experience is preferred, with CPR certification also

Qualifications

• Licensed PT with current California license.
• Experience in geriatric rehab preferred.
• CPR certification may be required.

Responsibilities

• Perform accurate patient evaluations and develop treatment plans.
• Supervise PTAs and aides in direct care per policy.
• Communicate patient progress with the team and families.
• Document evaluations, plans, progress, and discharges clearly.
• Coordinate with physicians, discharge planners, and staff.
