Job summary

Rockport Healthcare Services in California is seeking a licensed Physical Therapist to provide direct patient care in geriatric rehabilitation settings. You will evaluate patients, develop treatment plans, supervise assistants, and document progress in the medical records.

The role emphasizes interdisciplinary collaboration, safety, and compliance with facility policies; you will educate families and staff, participate in team conferences, and contribute to program quality improvements.

Qualifications

• Knowledge of physical therapy evaluation and treatments.
• Ability to supervise and direct assistants and aides.
• Understanding of physical modalities used in therapy.
• Knowledge of prosthetic/orthotic devices and assistive devices.
• Knowledge of medical terminology.
• Ability to work with all patient types.
• Excellent verbal and written communication skills.

Responsibilities

• Perform accurate, comprehensive patient evaluations.
• Develop effective plan of treatment and obtain approvals from referring physician.
• Supervise PTAs and aides in direct patient care and related activities.
• Communicate patient progress and treatment plans with supervisor and team.
• Instruct families and nursing staff in follow-up programs.
• Interview patients/families on functioning and recovery expectations.
• Maintain accurate documentation and compliance with regulations.
• Manage Medicare review and denials with leadership as needed.
