Job summary

Rockport Healthcare Services is seeking a Physical Therapist to provide direct patient care within a multidisciplinary therapy department in Oakland, CA. The role emphasizes advancing independence through evaluation, treatment planning, and collaboration with families and physicians.

Qualifications

• Experience in geriatric rehabilitation preferred.
• Experience in adult neurological, cardiac, and general rehabilitation is preferred.

Responsibilities

• Evaluate and treat patients and document services.
• Develop treatment plans and obtain physician approvals.
• Supervise Physical Therapy Assistants/other PTs and/or Aides in direct patient care.
• Communicate patient progress to supervisor and interdisciplinary team members.
• Instruct families and nursing staff in follow-through programs.
• Interview patients/families regarding prior functioning and expected outcomes.
• Demonstrate knowledge of resources for problem-solving and best practice.
• Record documentation that is accurate, complete and compliant with regulations.
• Contribute to rehab team conferences, care conferences, and utilization reviews.
