Job summary

Continuum Therapy Partners seeks a licensed Physical Therapist to join our Garner, NC location. You will evaluate patients, develop and implement individualized treatment plans, document progress, and educate families on home exercise programs.

Required is a Doctor of Physical Therapy (DPT) and an active state PT license with NPTE eligibility. One year of prior PT experience is preferred. Full-time daytime schedule at Hartwell Health and Rehab in Garner, NC.

Qualifications

• DPT degree required.
• Current, unencumbered state Physical Therapy License; must pass the NPTE.
• 1 year of previous experience as a Physical Therapist preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.

Physical Therapist Requirements & Responsibilities:
