Job summary

Continuum Therapy Partners is seeking a licensed Physical Therapist to join the Wayne Health & Rehab Center in Huber Heights, OH. You will evaluate patients, develop individualized treatment plans, and work with a multidisciplinary team to improve outcomes.

Qualifications

• Must hold a DPT degree and a valid state PT license.
• NPTE must be passed or in good standing for licensure.
• Typically 1 year of PT experience preferred.

• Educates patients and families on home exercises and injury prevention.

Physical Therapist Requirements & Responsibilities:

• Doctor of Physical Therapy (DPT) degree required

• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)

• 1 year of previous experience as a Physical Therapist preferred

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines

• Documents patient progress and adjusts plans of care based on clinical outcomes

• Educates patients and families on home exercise programs and injury prevention
