Job summary

COM seeks a Physical Therapist to join our interdisciplinary team delivering restorative therapy to participants in our Fresno facility and at-home visits. You will focus on care planning, independence, and safety, with lower patient volumes than typical outpatient settings.

You will participate in IDT meetings, document interventions, supervise therapy assistants, and train staff and families on equipment.

Qualifications

• Bachelor’s degree in physical therapy; master’s or doctoral degree preferred.
• California license to practice physical therapy or eligibility.
• CPR and First Aid certification within 180 days of hire.

Responsibilities

• Conduct PT assessments in our facility and occasionally in participants' homes.
• Participate in IDT meetings to develop participant care plans.
• Deliver and document interventions per plans: exercise, gait training, and modalities.
• Supervise Therapy Assistants and provide training for Functional Maintenance Programs.
• Educate staff, participants, and caregivers on equipment use and care.

At COM each participant of our program is guided by our interdisciplinary team (IDT) - composed of clinical and non-clinical members who work cohesively to provide quality patient-centered care.

The Physical Therapist is a key member of the IDT, focusing on care planning and the coordination and delivery of restorative and rehabilitative physical therapy services to participants, working to maximize independence and safety.

This role is different because Physical Therapists at COM:

• Lower patient volumes – no more seeing 3-4 patients an hour

On the day-to-day, you will:

• Conduct Physical Therapy assessments in our facility and occasionally in the home to define participant care needs, preferences, and goals
• Actively participate in IDT meetings to develop participant care plans
• Deliver and document interventions as agreed upon in participant care plansProvide active and passive exercise, muscle re-education, gait, functional training of ADLs, and treatments including massage, heat, sound, water, light, or electricity
• Supervise Therapy Assistants, providing training, instruction, and guidance needed to implement Functional Maintenance Programs (FMP)
• Provide training and instruction to staff, participants, and caregivers/families on the use and care of therapeutic equipment

Job requirements include:

• Bachelor’s degree in physical therapy; master’s or doctoral degree preferred
• Two (2) years in clinical setting with frail or elderly population preferred
• Valid California Physical Therapy License, or proof of license eligibility
• Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred
• We are seeking Physical Therapists that ideally have outpatient, acute, inpatient, or skilled nursing experience.
