Job summary

All Care and Comcare Home Health is seeking a compassionate Physical Therapist to join our growing team. This Field PT position offers you the ability to deliver high-quality, individualized care in patients' homes with a flexible schedule and supportive team environment.

Responsibilities include SOC/ROC/Recert/DC evaluations, documentation, goals alignment with the interdisciplinary team, and education for patients and caregivers on home exercise programs. Valid CA PT license required.

Qualifications

• Active California Physical Therapy license.
• Graduate of an accredited Physical Therapy program.
• Strong documentation and organizational skills.
• Excellent communication and interpersonal abilities.
• Willingness to travel to patient homes.
• Flexibility regarding work locations throughout service area.

Position: Field Physical Therapist

Key Responsibilities:

• Provide comprehensive, skilled physical therapy services and interventions in accordance with physician orders.
• Perform start of care (SOC) evaluations, resumption of care (ROC), recertifications (Recert), and discharges (DC).
• Assess patient skilled needs, plan interventions, evaluate outcomes, and modify care to meet physical therapy goals.
• Collaborate with the interdisciplinary team to develop and evaluate individualized patient goals.
• Educate, instruct, and train patients, families, and caregivers on physical therapy programs and home exercise regimens.

Qualifications:

• Active license/registration with the Physical Therapy Board of California.
• Graduate of an accredited Physical Therapy program.
• Strong documentation and organizational skills.
• Excellent communication and interpersonal abilities.
• Willingness to travel to patient homes for evaluations and treatment.
