Job summary

All Care and Comcare Home Health is seeking a compassionate Field Physical Therapist to join our growing team. This role offers a flexible schedule and the opportunity to provide high-quality, individualized care in patients’ homes.

The Physical Therapist will perform SOC evaluations, plan interventions, and educate patients and families while collaborating with an interdisciplinary team. Travel is required within our service area, with a strong focus on outcomes and patient-centered care.

Qualifications

• Active license/registration with the Physical Therapy Board of California.
• Graduate of an accredited Physical Therapy program.
• Strong documentation and organizational skills.
• Excellent communication and interpersonal abilities.
• Willingness to travel to patient homes for evaluations and treatment.
• Flexibility regarding work locations throughout our service area.

Responsibilities

• Provide comprehensive, skilled physical therapy services and interventions in accordance with physician orders.
• Perform start of care (SOC) evaluations, resumption of care (ROC), recertifications (Recert), and discharges (DC).
• Assess patient skilled needs, plan interventions, evaluate outcomes, and modify care to meet physical therapy goals.
• Collaborate with the interdisciplinary team to develop and evaluate individualized patient goals.
• Educate, instruct, and train patients, families, and caregivers on physical therapy programs and home exercise regimens.
