Job summary

All Care and Comcare Home Health in California is seeking a compassionate Field Physical Therapist to join our growing home health team. You will provide skilled PT services at patients' homes with a flexible schedule.

Responsibilities include SOC/ROC/recert/discharge evaluations, developing individualized goals, educating patients and families, and collaborating with the interdisciplinary team to ensure optimal outcomes.

Qualifications

• Active California Physical Therapy license/registration.
• Graduation from an accredited Physical Therapy program.
• Strong documentation and organizational skills.
• Excellent communication and interpersonal abilities.
• Willingness to travel to patient homes.

Position: Field Physical Therapist

Key Responsibilities:

• Provide comprehensive, skilled physical therapy services and interventions in accordance with physician orders.
• Perform start of care (SOC) evaluations, resumption of care (ROC), recertifications (Recert), and discharges (DC).
• Assess patient skilled needs, plan interventions, evaluate outcomes, and modify care to meet physical therapy goals.
• Collaborate with the interdisciplinary team to develop and evaluate individualized patient goals.
• Educate, instruct, and train patients, families, and caregivers on physical therapy programs and home exercise regimens.

Qualifications:

• Active license/registration with the Physical Therapy Board of California.
• Graduate of an accredited Physical Therapy program.
• Strong documentation and organizational skills.
• Excellent communication and interpersonal abilities.
• Willingness to travel to patient homes for evaluations and treatment.
• Flexibility regarding work locations throughout our service area.
